Psoriasis in the hairline: what should you know?

The hairline is a common boundary for scalp psoriasis, so a plaque may be visible where the scalp meets the forehead or neck. It can look red, darker than surrounding skin, raised or covered with silvery scale. Itch, tightness and flaking are possible, but symptoms vary. Hair products, dyes, sweat and friction from headwear can aggravate the area. A patch at the hairline may also be seborrhoeic dermatitis, eczema, contact dermatitis or another rash. The direction of spread and the appearance of scale help a clinician assess the cause; a picture alone is not enough for a diagnosis.

Clean the area gently and pat it dry. Keep styling products and hair dye off irritated skin, and test or stop a product if burning begins. Do not scrape scale or scrub the forehead. Use only treatment recommended for the specific site, because skin near the face is more sensitive than the scalp and some scalp medicines are unsuitable there. Arrange a review if the hairline plaque persists, enlarges, repeatedly cracks or interferes with sleep or daily care. Seek prompt advice for rapid swelling, warmth, pus, severe pain, fever, marked facial involvement or eye symptoms. A rash that starts soon after hair dye or a new product may need evaluation for contact irritation. Sudden hair loss, bleeding after scratching, or lesions spreading behind the ears or down the neck also warrants a clinician’s examination.
