Psoriasis at the base of the scalp: what to know

The base of the scalp includes the hair-bearing skin above the neck and often the areas behind the ears. Psoriasis there may form a thickened plaque with dry scale, itch, redness, tenderness, or cracks. Sweat, collars, helmets, and hair rubbing against the neck can make discomfort more noticeable. Scale may cling to hair and become harder to remove without softening. A rash in this region can also come from irritation, allergy, infection, or eczema, so its appearance alone cannot confirm the cause.

Keep the area clean with gentle washing and dry it by patting. Avoid picking under scale or using a sharp comb to lift it. If a clinician recommends a softening product, allow it time to work before washing rather than pulling at the plaque. Apply prescribed lotion, solution, or foam to the affected skin using a parted-hair approach. Choose breathable collars and loosen headwear during flares when possible. Do not share combs, brushes, or towels if the skin is broken.

Arrange a review when pain, itch, or scale persists, returns frequently, or prevents comfortable sleep. Prompt assessment is appropriate for rapidly increasing redness, warmth, swelling, pus, fever, or a painful tender lump. New hair loss, broken hairs, or a spreading patch may indicate a different scalp problem and should not be self-treated as psoriasis. Also tell the clinician about nail changes, swollen fingers, or prolonged joint stiffness.
