What should you know about pseudogout rhomboid crystals?

During joint-fluid microscopy, calcium pyrophosphate crystals may look short, blunt or rhomboid-shaped. They have weak positive birefringence under polarized light, a technical feature that helps distinguish them from the needle-shaped crystals associated with gout. In the right setting, this finding supports calcium pyrophosphate deposition disease, also called pseudogout.

The result still needs to be interpreted with the examination. A swollen joint can have more than one cause, and joint fluid may also be checked for white cells, bacteria and other clues. Finding crystals does not make a fever or severe illness safe to ignore.

Ask which joint was sampled, whether the laboratory identified calcium pyrophosphate crystals, and whether infection testing was performed. Follow the prescribed plan for symptom control. Depending on your health history and the joint involved, treatment may include anti-inflammatory medication, joint aspiration or a cortisone injection.

Contact urgent medical services for a hot, swollen joint with fever, chills, confusion, rapidly worsening pain or inability to move the limb. These symptoms can indicate infection, which can occur alongside a crystal finding and needs rapid attention.

Ask how confident the result is, whether the fluid was cultured, whether another joint could be involved, and which medication precautions apply to your kidney, stomach or heart health.
