Understanding prostate cancer biopsy results

A pathology report may describe samples from different areas of the prostate. It can say that a sample is benign, shows inflammation, or contains cancer. When cancer is present, the report may include a grade or Grade Group, which reflects how abnormal the cells appear under a microscope. The result is interpreted alongside your PSA pattern, examination and scans; a biopsy report alone does not describe the whole situation.

A negative sample does not always settle the question if other findings remain concerning, because a biopsy examines selected tissue. Your specialist may discuss monitoring, another biopsy, imaging or treatment depending on the complete picture.

Ask for a copy of the pathology report and request an explanation of every unfamiliar term. Bring your PSA history, medicines and previous scan reports to the follow-up visit. Possible plans include active surveillance for a cancer considered suitable for close monitoring, surgery, or radiation-based treatment. Options such as radiotherapy, brachytherapy, prostate removal surgery and robotic cancer surgery have different preparation, recovery and side-effect considerations.

After a biopsy, contact your clinical team promptly for fever, chills, worsening pain, heavy bleeding, inability to pass urine, or feeling suddenly very unwell. Difficulty passing urine can require urgent assessment, especially when the bladder feels full or painful. Do not wait for the routine results appointment if these symptoms develop.

Ask: Were cancer cells found in every sample or only some? What are the grade and Grade Group? Does the report suggest cancer is confined to the prostate? Do I need an MRI or other scan? Which options fit my health and priorities, and what side effects should I plan for?
