What should you know about prom labor?

Short answer

PROM labour refers to labour after the waters break before contractions begin, so maternity teams assess timing, infection, and the baby's wellbeing.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

PROM and labour

PROM is premature rupture of membranes: the fluid-filled sac opens before labour has started. Labour may begin later, or contractions may develop soon after the leak. The approach depends on pregnancy stage, the baby's position and movements, fluid colour and smell, your observations, and signs of infection. When the rupture happens before the pregnancy is ready for birth, it can lead to preterm labour. A gush is not required; some people notice only ongoing dampness.

Steps after suspected PROM

Contact your maternity unit immediately and explain when the leaking began, whether it is continuous, and whether contractions or bleeding are present. Wear a pad, keep the fluid available for assessment if possible, and avoid tampons, sex, or inserting anything vaginally until instructed. Clinicians may confirm the rupture, examine safely, monitor the fetal heartbeat, and discuss waiting for labour or planning birth. An [Antenatal Steroid Injection](/treatments/pregnancy-and-birth-care/antenatal-corticosteroid-injection/) may be offered when early birth is possible. [Induction of Labour](/treatments/pregnancy-and-birth-care/induction-of-labour/) is one possible birth-planning option.

When labour symptoms need urgent attention

Go to urgent maternity care for regular painful contractions, fever or chills, abdominal tenderness, bleeding, green or unpleasant-smelling fluid, reduced fetal movements, or a sudden feeling that something is wrong. A cord-like structure at the vaginal opening is an emergency: call emergency services, avoid pushing it back, and follow instructions while waiting. Suspected PROM needs assessment even if contractions have not started.

Questions to ask

Ask how labour will be recognised, how the baby will be monitored, what would make induction safer than waiting, and what pain-relief choices are available. Clarify whether you need to stay in hospital, how infection will be checked, what fetal movement pattern to report, and who to call if the leak or contractions change.

Questions people ask

Can labour happen after PROM?

Yes. Contractions may start after the waters break, but the timing varies and should be managed with maternity guidance.

What should I do if contractions start after leaking?

Contact your maternity unit, describe the contraction pattern and fluid, and follow its instructions for assessment and travel.
