What should you know about prognosis for stage 4 lung cancer?

Stage 4 lung cancer has spread beyond the place where it started. It may involve the other lung, distant lymph nodes, organs or the brain. The exact sites, number and size of deposits, and the symptoms they cause help the team choose treatment and discuss goals.

The outlook is individual. Pathology identifies the lung cancer type, while genomic tumour profiling may find changes that guide targeted treatment. Scans and follow-up show whether disease is responding, stable or changing. These results can lead to a different plan over time.

Ask whether genomic testing is complete and which treatment options fit the tumour findings and your health. Depending on the situation, care may include chemotherapy, radiotherapy or another systemic treatment. Surgery such as a lung lobectomy is considered only in selected circumstances, rather than for every stage 4 diagnosis.

Palliative care can begin alongside cancer treatment. It can help with breathlessness, pain, cough, fatigue, sleep, appetite and emotional or practical concerns. Tell the team about hoarseness, worsening cough or any new symptom so it can be assessed rather than attributed automatically to cancer.

Get emergency help for severe breathlessness at rest, blue lips, chest pain with collapse, coughing large amounts of blood, sudden confusion or new one-sided weakness. Contact the oncology team promptly for fever, rapidly worsening breathlessness, inability to keep fluids down or new severe pain.

Ask where the cancer has spread, whether genomic profiling is complete, what each treatment is intended to do, how response will be checked, and which symptoms require an urgent call.
