What primary insomnia involves
Sleep trouble may occur even when you have enough opportunity and a suitable place to sleep. You might lie awake, wake repeatedly, or start the day earlier than intended, followed by tiredness, irritability, or Difficulty Concentrating. Daytime Sleepiness can affect driving and work. Stress, a changing routine, travel, caffeine, alcohol, pain, medicines, Depression, Generalised Anxiety Disorder, Burnout, or Jet Lag may contribute. The word primary means no other known condition fully accounts for the sleep problem after assessment; it is not a judgement about how hard you are trying.
Building better sleep conditions
Keep a steady wake time, including after a poor night, and seek morning daylight. Use the bed for sleep rather than prolonged worrying or scrolling. If you remain awake and frustrated, get up briefly for a quiet activity in dim light, then return when sleepy. Keep the room cool, dark, and quiet; limit late caffeine, nicotine, heavy meals, and alcohol. A sleep diary can show patterns. Cognitive behavioural therapy for insomnia can address unhelpful sleep thoughts and habits. Check with a clinician before using sleeping tablets or supplements, particularly with other medicines.
When to seek medical help
Arrange an appointment when sleep difficulty persists, repeatedly disrupts daytime function, or leads to reliance on sedatives. Seek urgent help if you feel unsafe, have thoughts of harming yourself, or are too sleepy to drive safely. Tell a clinician about loud snoring, breathing pauses, unusual night movements, mood changes, pain, or substance use because these clues may point to another sleep or health problem.
Questions for your doctor
