Preventive cervical cerclage: what to know

The cervix is the lower part of the womb. In some pregnancies it may shorten or open before the baby is ready to be born. A preventive cerclage is usually discussed because of a previous late miscarriage, preterm birth or known cervical weakness, rather than because symptoms prove that early labor is happening.

The procedure places a strong suture around the cervix through the vagina, commonly during early or middle pregnancy according to the individual's history and scan findings. It does not treat every cause of preterm labor and the decision balances possible benefit with procedure-related concerns.

Discuss your pregnancy history, bleeding, fluid loss, contractions, allergies and medicines before the procedure. Your team may arrange an examination or ultrasound and explain the anaesthetic, aftercare and activity advice. Mild cramping or spotting can occur afterward; follow the discharge instructions and attend planned reviews.

Ask before intercourse, travel, exercise or swimming, since restrictions vary with symptoms and clinical findings. Keep the maternity unit's contact details nearby. The stitch is generally removed later in pregnancy or earlier if labor starts or the waters break, according to your obstetric team's plan.

Contact your maternity unit urgently for regular contractions, worsening cramps, vaginal bleeding, a gush or persistent leak of fluid, fever, foul-smelling discharge or severe pelvic pain. If labor or membrane rupture is suspected, the stitch may need prompt review; do not wait for a routine appointment.
