Pregnancy and a swollen leg: what should you know?

Short answer
Mild swelling in both legs can occur during pregnancy, but sudden or one-sided swelling, pain, warmth, or breathlessness needs urgent medical assessment.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Often less concerning
- Mild swelling affecting both legs and worse later in the day.
- Urgent sign
- One-sided pain, warmth, or rapid swelling.
- Emergency sign
- Breathlessness or chest pain with leg swelling.
Understanding leg swelling
Pregnancy can increase fluid retention and make swelling more noticeable later in the day, especially around the ankles. Standing for long periods, heat, and reduced movement may add to it. Swelling that is mainly on one side, develops quickly, or comes with calf tenderness can signal a blood clot. Marked swelling with headache, vision changes, or upper abdominal pain can accompany a pregnancy blood-pressure problem.
Practical steps
Rest with your legs supported, avoid standing still for long periods, move your ankles gently, and wear comfortable shoes. Drink normally and avoid taking water tablets or supplements for swelling unless prescribed. Note whether both legs are affected, when swelling appears, and whether skin colour, warmth, or pain changes. Mention recurring swelling at antenatal appointments.
When to seek care
Seek emergency care for sudden breathlessness, chest pain, coughing blood, fainting, or a rapidly painful, warm, one-sided leg. Contact your maternity team urgently for sudden marked swelling, severe headache, vision changes, or pain beneath the ribs. Arrange review for persistent swelling, skin changes, or symptoms that interfere with walking.
Questions for your doctor
Ask whether your blood pressure, urine, and leg need checking, whether compression is suitable, and which activity is safe. Ask how to distinguish ordinary ankle swelling from a clot or another condition.

Questions people ask
Is swelling in both ankles common during pregnancy?
It can occur from fluid retention, heat, standing, or reduced movement, but sudden or marked change should be reported.
Can I massage a swollen leg during pregnancy?
Do not massage a painful, warm, or one-sided swollen leg before urgent assessment, because a clot needs to be considered.
