Pregnancy-induced heart failure

Heart failure means the heart is not pumping or filling well enough for the body’s needs. It can develop during pregnancy or after delivery, when the circulation is under extra demand. Breathlessness, unusual fatigue, swelling of the ankles, cough when lying down, or a racing or irregular heartbeat can be warning symptoms, although some overlap with normal pregnancy changes.

The symptoms themselves do not confirm heart failure. Hypertension, an abnormal rhythm such as atrial fibrillation, anaemia, lung disease, and other problems may produce similar complaints. Assessment can include an examination, blood tests, an electrocardiogram, and heart imaging, with the plan adapted to pregnancy or breastfeeding.

Tell your maternity team promptly about new or worsening breathlessness, swelling, fatigue, cough, or palpitations. Keep appointments and take prescribed medicines exactly as directed; some heart medicines need changing before pregnancy, during pregnancy, or while breastfeeding. Do not start, stop, or alter treatment without the specialist who knows your situation.

Your team may review blood pressure, weight changes, fluid balance, symptoms, and heart function. Rest when breathless, avoid smoking, and follow personalised advice about activity, salt, and fluids. A chronic disease review can help coordinate follow-up after the immediate assessment.

Call emergency services for severe breathlessness at rest, inability to lie flat because of breathing, chest pain, fainting, confusion, blue or grey lips, coughing pink or frothy sputum, or a sudden major deterioration. Contact your maternity team urgently for rapidly increasing swelling, persistent palpitations, or breathlessness that is new or worsening.
