Pregnancy and leg swelling: what to know

Short answer
Leg swelling in pregnancy is often linked to fluid shifts and pressure on veins, but sudden, one-sided, painful, or rapidly worsening swelling needs urgent assessment.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

Why legs may swell
Pregnancy changes how fluid moves through the body. The growing uterus can press on pelvic veins, making fluid collect around the feet and ankles, especially after standing or later in the day. Swelling that affects both sides gradually and improves with rest is often less concerning.
A single swollen leg with warmth, redness, tenderness, or a heavy feeling can signal a clot and requires prompt assessment. Swelling with headache, vision changes, upper abdominal pain, or high blood pressure symptoms also needs urgent maternity advice.
Reducing everyday discomfort
Change position regularly, avoid standing for long periods, and rest with your legs supported. Comfortable shoes and loose clothing can reduce pressure. Gentle ankle movements and walking may help circulation if your maternity team says activity is suitable.
Do not take water tablets or use tight compression garments without individual advice. Mention swelling at routine appointments, particularly if it is increasing. A clinician can consider other contributors, including [Varicose Veins](/conditions/varicose-veins/) or kidney and heart conditions.
When to seek medical care
Get urgent help for one-sided swelling with pain, redness, or warmth; sudden swelling of the face or hands; severe headache; visual changes; chest pain; difficulty breathing; coughing blood; or fainting. Do not wait for a routine appointment when these symptoms appear.
Questions for your doctor
Ask whether the pattern of swelling is expected, whether your blood pressure and urine need checking, and which activity or support is appropriate. Ask what changes should prompt immediate contact and whether your medicines could affect fluid balance.

Questions people ask
Is ankle swelling common during pregnancy?
Gradual swelling on both sides can occur because pregnancy affects fluid movement and venous pressure. Your maternity team should assess changes that are sudden, severe, or unusual for you.
Should I reduce my fluid intake?
No. Restricting fluids can be harmful. Drink according to your clinician's advice and seek assessment if swelling comes with illness or other warning symptoms.
