Pre-eclampsia vision changes: what they mean and what to do

Pre-eclampsia is a pregnancy-related condition involving high blood pressure and possible effects on organs. Changes in vision may happen when the condition affects the nervous system or the blood flow around the eyes and brain. Symptoms can feel like hazy or blurred sight, seeing spots or sparkles, flashing lights, difficulty focusing, double vision, or a dark area in the field of vision. A severe headache, pain under the ribs, sudden swelling, nausea, or feeling unusually unwell can occur alongside visual symptoms.

Visual symptoms can have causes unrelated to pre-eclampsia, but they cannot be safely sorted out at home during pregnancy or soon after birth. A blood-pressure reading and an assessment of your symptoms help the maternity team decide what monitoring or treatment is needed.

Stop driving and contact your maternity unit, obstetrician, or local emergency service as soon as vision changes begin. Tell them that you are pregnant or recently gave birth, describe exactly what you can see, and mention any headache, abdominal pain, breathlessness, or reduced movement of the baby. If you have a home blood-pressure monitor, you may take a reading while arranging help, but do not delay assessment to repeat it or wait for the symptom to pass. Take your pregnancy records and a list of medicines if this is practical.

At assessment, clinicians may check your blood pressure, urine, blood tests, reflexes, and symptoms, and may monitor your baby. Treatment depends on the findings and how far along the pregnancy is; it can include medicines, observation, and planning birth when that is safest.

Seek emergency help immediately for sudden or worsening vision loss, flashing lights with a severe headache, confusion, a seizure, fainting, severe shortness of breath, chest pain, or severe pain in the upper abdomen. Do the same if you feel seriously unwell after delivery, because pre-eclampsia can first appear or continue after birth. Do not wait for a routine antenatal appointment.

Even mild or brief visual changes deserve prompt advice from a maternity professional. Keep the room safe, sit or lie on your side if you feel faint, and have someone stay with you while help is arranged.
