Why late pregnancy is monitored closely

Pre-eclampsia can develop after a previously uncomplicated pregnancy. It may cause high blood pressure, protein or other abnormalities in urine, and changes in organs such as the kidneys, liver or brain. The placenta may not deliver blood as effectively, which can affect fetal growth or movement. Some people feel well, so routine antenatal checks remain useful. A headache that persists, visual symptoms, pain beneath the ribs, sudden swelling or breathlessness deserves medical attention rather than watchful waiting at home.

What assessment and planning involve

Your team may repeat blood-pressure readings, test urine and blood, and use fetal heart monitoring or ultrasound to assess growth, fluid and blood flow. The plan depends on your results, symptoms, gestational age and the baby's condition. It may include closer review, hospital observation, blood-pressure medicine, or medicine to reduce seizure risk. If remaining pregnant becomes less safe, the team may recommend birth by induction or Caesarean Section. Continue prescribed treatment and attend every review.

Warning signs in the third trimester

Contact maternity services urgently for a severe or persistent headache, blurred vision or flashing lights, severe upper-abdominal pain, sudden breathlessness, chest pain, confusion, a seizure, vaginal bleeding or reduced fetal movement. Do not wait for the next antenatal appointment. If you are unsure whether a symptom is significant, call the maternity service for immediate guidance.
