PPROM expectant management: what it involves

Expectant management is a monitored plan, not simply waiting at home. The maternity team considers gestational age, the amount and appearance of leaking fluid, your observations, blood results, contractions and the baby’s heart rate. Ultrasound may be used to assess fluid and growth, but clinical assessment remains important.

The aim is to allow more time for fetal development when there is no immediate reason for birth. The plan can change at any moment if infection, labour, bleeding, fetal distress or another concern appears. Your hospital may recommend staying in or returning frequently for review, depending on your circumstances.

Follow the unit’s instructions about activity, hygiene, medicines and monitoring. Avoid vaginal intercourse, tampons and inserting products unless your clinician specifically advises otherwise. Report your temperature, pain, contractions, bleeding, fluid colour or smell, and fetal movement as directed. Contact the maternity unit urgently for fever, chills, new or worsening abdominal pain, uterine tenderness, regular contractions, bleeding, offensive-smelling or green fluid, or reduced fetal movement. These symptoms can indicate infection, labour, bleeding or fetal compromise. Use the emergency service if you are very unwell, faint, or unable to reach maternity care promptly.
