PPROM at 17 weeks: what to know

PPROM is a break in the amniotic sac before labour and before the pregnancy reaches term. At 17 weeks, fluid leakage may be subtle or continuous, and the diagnosis may require examination and testing. The clinical team considers infection, bleeding, pain, the amount of remaining fluid and the wellbeing of the pregnant person. The pregnancy is very early, so counselling should cover uncertainty, emotional support and the possible effects of prolonged membrane rupture.

Contact your maternity service immediately if you suspect watery leakage; do not wait for a routine appointment. Use a clean pad to observe fluid, avoid putting anything in the vagina, and follow instructions about examination and monitoring. Do not self-start antibiotics or other medicines. Ask which symptoms require returning to hospital and who to contact outside clinic hours. Management may involve close observation or discussion of ending the pregnancy, depending on findings and your informed preferences.

Go to urgent maternity care for fever or chills, worsening abdominal pain, uterine tenderness, vaginal bleeding, foul-smelling fluid, dizziness, or feeling suddenly unwell. Reduced wellbeing or new contractions also need immediate review. Infection can become serious, so symptom changes should be reported promptly.
