Tight neck and shoulder muscles, an awkward sleeping position, long periods looking down, stress, missed meals, and poor fluid intake can all contribute to pain felt at the back of the head. The pain may be pounding, pressing, or accompanied by tenderness in the scalp or neck.

Heat exposure and fluid loss can bring on headache with thirst, tiredness, dizziness, or reduced urination. Nasal congestion from a cold, allergies, or sinus problems can also occur alongside head pressure, although a back-of-head headache does not by itself establish any of these conditions.

Pause strenuous activity, move to a quiet cool place, and drink fluids if you may be dehydrated. Eat a regular meal if you have not eaten, and try gentle neck movement rather than forcing a painful stretch. A warm or cool compress on the neck may feel soothing.

If you use a pain medicine, follow the product label and check with a pharmacist or clinician if you are pregnant, have stomach, kidney, liver, bleeding, or heart problems, or take other medicines. Repeatedly relying on pain medicine for headaches is a reason to arrange a medical review.

When to get medical help

Seek emergency care for a headache that reaches maximum intensity suddenly, follows a head injury, or comes with fainting, confusion, seizure, fever with a stiff neck, new weakness or numbness, trouble speaking, loss of balance, or a major change in vision.

Arrange a prompt appointment if the pattern is new, keeps returning, is getting worse, wakes you from sleep, or occurs during pregnancy or after giving birth. Bring a note of triggers, timing, associated symptoms, and medicines you have taken.
