Why swelling can happen after birth

Pregnancy-related fluid, changes in circulation, time spent sitting, and intravenous fluids during labour or a caesarean birth can leave the feet, ankles, and lower legs puffy. Swelling may be more noticeable later in the day and may affect both legs. It should gradually settle as your body moves excess fluid back into circulation and removes it.

Practical measures that may help

Take short, easy walks around the room and flex your ankles while resting. Prop your legs on pillows so your feet are raised, avoid crossing your legs for long periods, and choose loose clothing and supportive footwear. Drink according to thirst unless your clinician has given you a fluid restriction. Do not start water tablets, herbal remedies, or compression garments without checking first, especially after surgery or if you have high blood pressure.

Compare both legs once or twice a day rather than repeatedly checking. Note whether swelling is improving, whether one side is warmer or more tender, and whether your shoes or rings become suddenly tight. These details help a clinician decide whether you need an examination.

When to seek care

Get urgent help for swelling in one leg with pain, warmth, redness, or tenderness, or for chest pain, sudden breathlessness, coughing blood, faintness, or a racing heartbeat. Contact your maternity team or clinician promptly for a severe headache, vision changes, upper-abdominal pain, marked facial or hand swelling, or a high blood-pressure reading. Seek review if swelling is worsening, does not begin to ease, or comes with fever, a wound problem, or reduced urination.
