What should you know about postmenopausal endometrial thickening without bleeding?

A thickened endometrium without bleeding is an ultrasound finding, not a diagnosis, and its significance depends on the scan, medicines and clinical context. The womb lining can look different because of a polyp, hyperplasia, hormone exposure or, less commonly, cancer. The absence of bleeding is reassuring in some situations but does not decide whether follow-up is needed.

The measurement is interpreted alongside the quality of the scan, the appearance of the lining and whether it looks smooth or contains a focal area. Endometrial Polyps can create a local thickening, while Endometrial Hyperplasia means the lining cells have grown in an abnormal pattern. Womb cancer can present with bleeding, but may occasionally be found during investigation for another reason. Medication such as oestrogen or tamoxifen can also affect interpretation.

Ask the clinician who arranged the scan what was measured and whether the finding was diffuse or localised. A gynaecologist may recommend a repeat scan, hysteroscopy or a biopsy depending on your history and the scan appearance. Tell the team about hormone therapy, tamoxifen, previous bleeding, pelvic procedures and any family history they ask about. Do not start or stop prescribed treatment without advice.

Report any new vaginal bleeding, spotting, watery discharge, pelvic pain or bleeding after sex, even if the scan was found by chance. Seek urgent help for heavy bleeding, faintness, severe pain or fever. A symptom-free result still deserves the follow-up your clinician recommends, because the scan cannot identify every cause on its own.

Ask for the measured lining thickness, the scan’s description, whether your medicines affect it and why a biopsy or repeat imaging is being suggested. Clarify when you should expect results and which symptoms should prompt earlier contact.
