Pneumonia or heart failure symptoms

Pneumonia is an infection that may bring fever, chills, phlegm and a sudden change in breathing. Heart failure can cause breathlessness with activity or when lying flat, fatigue, palpitations and swelling around the ankles as fluid builds up. These patterns are clues, not a diagnosis: a person can have both conditions, and other problems can cause the same symptoms. A clinician may review your history, examine your chest and legs, and assess heart and lung function.

Contact your healthcare team when breathlessness, cough, fatigue or ankle swelling is new, persistent or changing. Continue prescribed heart medicines unless a clinician tells you otherwise, and record changes in weight, sleep position, temperature and swelling to discuss at review. Do not use another person’s water tablets or antibiotics. A chronic disease review can help check medicines and agree on a plan for future flare-ups.

Seek emergency help for severe breathlessness at rest, chest pressure, fainting, new confusion, blue or grey lips, or inability to stay awake. Rapidly increasing swelling, sudden palpitations with dizziness, or coughing pink frothy fluid also requires urgent assessment. A new fever with worsening breathlessness should be reviewed promptly because infection can place extra strain on the heart.

Ask whether the pattern fits infection, fluid build-up or both; whether tests are needed; how to adjust activity and medicines; and which changes should trigger emergency care.
