Pneumonia and heart failure: what to know

Pneumonia inflames the lungs and can make oxygen exchange harder. The body may respond with a faster heartbeat, while illness, reduced activity, or changes in fluid balance can add work for a heart already struggling to pump effectively. Cough and fatigue may belong to either condition. Increasing breathlessness, needing more pillows, rapid weight change, palpitations, or newly swollen ankles may signal worsening heart failure, pneumonia, or both.

Contact your heart-failure team or a clinician promptly about a new cough, fever, breathlessness, or swelling. Keep taking prescribed medicines unless a clinician tells you otherwise, and follow your existing fluid and salt guidance rather than changing it yourself. Bring an up-to-date medicine list and describe how symptoms compare with your usual baseline. Assessment may include listening to the chest, checking oxygen and heart rhythm, and reviewing fluid status.

Call emergency services for severe breathlessness at rest, blue or grey lips, fainting, confusion, severe chest pressure, or rapidly worsening swelling with difficulty breathing. A very fast or irregular heartbeat with weakness or chest discomfort also needs urgent attention. Do not drive yourself if you feel faint or cannot breathe comfortably.
