Plus-size compression garments: what to know

Graduated compression is usually firmer at one end of the limb and less firm higher up. This can support movement of fluid through tissues when lymphatic drainage is impaired. A larger size does not automatically mean a looser fit: the garment should follow your measurements without creating a tight band. Swelling can change during the day, after surgery, or during cancer treatment, so fit may need reassessment.

Have the limb measured when swelling is stable if possible, and ask how long to wear the garment and how to put it on without folding the fabric. Check the skin after removing it, wash it according to the care instructions, and replace it when the fabric loses its shape. Lymphoedema therapy may add skin care, movement, and other techniques. Do not alter the garment yourself if it causes pressure points.

Remove the garment and seek clinical advice for new numbness, colour change, coldness, severe pain, blistering, or marked skin irritation. Sudden swelling with warmth, redness, tenderness, or fever needs prompt assessment because infection or a blood-flow problem may require different care. Urgent help is needed for sudden breathlessness or chest pain.

Ask which limb measurements should be recorded, what pressure and garment shape are appropriate, and whether you need a custom-made option. Discuss how to manage swelling if the garment becomes uncomfortable, how often it should be reviewed, and whether exercise or lymphoedema therapy should accompany it.
