What the finding means

Pleural effusion is a collection of fluid in the space around a lung. It can cause breathlessness, cough, chest discomfort, or difficulty lying flat. In a person with ovarian cancer, the fluid may be connected with cancer, treatment, infection, heart or kidney problems, or another medical issue; the symptom alone cannot identify the cause.

Evaluation may include examination, chest imaging, blood tests, and drainage of fluid for laboratory analysis. The result is interpreted with pelvic findings, abdominal symptoms, scan changes, and the existing ovarian cancer diagnosis. A pelvic ultrasound looks at pelvic structures but does not replace chest assessment.

What to do next

Tell the oncology team when breathlessness began and whether it occurs at rest, with walking, or while lying down. If the effusion is large or uncomfortable, drainage may relieve pressure and provide fluid for testing. The treatment plan may include chemotherapy, surgery such as ovarian cancer debulking surgery, or another approach based on the cancer’s extent, prior treatment, genomic tumour profiling, and overall health.

BRCA genetic testing may be relevant to wider ovarian cancer treatment discussions. Palliative care can support breathlessness, pain, fatigue, appetite, and anxiety alongside cancer-directed care.

When to seek care

Get urgent help for sudden severe breathlessness, chest pain, fainting, confusion, blue lips, or inability to speak comfortably. Contact the treating team promptly for steadily worsening breathing, fever, new severe weakness, or increasing pain after fluid drainage. These symptoms require assessment and should not be judged by the presumed cause of the effusion.
