Pink Frothy Sputum and Heart Failure: What to Know

This symptom may occur when the heart cannot pump effectively enough, allowing pressure and fluid to move into the lung tissue. It can also have other causes, so a clinician needs to assess you rather than relying on sputum colour alone.

Sputum is mucus brought up from the airways. A pink, foamy or frothy appearance can reflect a mixture of fluid and a small amount of blood in the lungs. In acute heart failure, this is sometimes linked with pulmonary oedema, which can make breathing suddenly difficult.

Heart failure may also cause breathlessness when lying down, waking at night short of breath, tiredness, a cough, or swelling around the ankles. Atrial fibrillation and hypertension can contribute to heart strain, but pink sputum does not by itself confirm any particular diagnosis.

If you are bringing up pink frothy sputum now, sit upright, keep activity to a minimum, and seek emergency medical help. Do not drive yourself if you are struggling to breathe, faint, confused, or very weak. If you have a prescribed heart-failure action plan, follow its emergency instructions; do not take extra medicine unless a clinician has specifically told you to do so.

Take your current medicine list and tell the medical team about known heart or lung conditions, missed doses, recent illness, chest discomfort, and any rapid change in weight or swelling. Assessment may include oxygen checks, an examination, an electrocardiogram, and imaging or blood tests.

Call emergency services immediately for pink frothy sputum with severe or rapidly worsening breathlessness, blue or grey lips, chest pain, collapse, new confusion, or an inability to speak in full sentences. These signs can indicate a serious problem with oxygen and circulation.

Even if the breathing settles, arrange prompt medical review because the trigger may need treatment and your heart-failure plan may need adjustment. A new cough without pink frothy sputum still deserves advice if it comes with worsening breathlessness, ankle swelling, palpitations, or unusual fatigue.
