A pilar cyst is a capsule connected to a hair-follicle structure. As long as that lining remains, it can continue to hold keratin material, so the lump may remain even when it is not painful. It can also become less noticeable for a period and then swell again. A sudden change in size or tenderness may reflect inflammation or rupture rather than the cyst resolving.

If a clinician has identified a small, quiet cyst, monitoring can be reasonable: note its size, avoid squeezing it, and protect it from combing or other friction. Book a review if it becomes troublesome or changes. If removal is chosen, a clinician removes the cyst and aims to address its capsule; timing may depend on whether the area is currently inflamed. Home drainage does not reliably remove the capsule.
