Physical Therapy for a Hamstring Tear

Short answer

Physical therapy for a hamstring tear protects healing tissue first, then progressively restores flexibility, strength, coordination and confidence with movement.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Early priority
Protect healing tissue while maintaining safe movement
Later priority
Progressive strength, speed, coordination and task-specific control

Recovery is staged

Assessment looks at the injury location, bruising, tenderness, walking pattern, hip and knee movement, and strength compared with the other side. Early care may limit painful stretching and high-speed activity while comfortable walking and gentle muscle activation are maintained. Later work can include bridges, controlled hamstring loading, hip and trunk strength, balance, running drills and sport-specific changes of direction. The pace depends on symptoms, tear severity, activity demands and whether the tendon attachment is involved.

Loading the hamstring carefully

Follow the staged programme and increase only one factor at a time, such as resistance, range, speed or distance. Warm up as advised, avoid sudden sprinting while strength and control are incomplete, and use a comfortable stride during early walking. Mild exercise effort is different from a sudden pull, sharp pain or growing bruising. Report those changes instead of testing the muscle with a harder session. Return to sport should include the movements your sport requires, not only pain-free walking.

When to seek medical care

Arrange prompt assessment for severe pain, a visible hollow or pop at the back of the thigh, extensive bruising, inability to walk normally, or weakness that is not improving. Seek urgent help for a cold or pale foot, marked calf swelling, chest pain or difficulty breathing. New symptoms after returning to fast activity may signal that loading has advanced too quickly.

Questions to discuss

Ask whether the tear is in the muscle or near its tendon, which stretches and loads are currently allowed, and what strength test guides progression. Discuss work or sport demands, running drills, pain-monitoring rules and the signs of reinjury.

Questions people ask

Can I stretch a hamstring tear immediately?

Forceful stretching can irritate healing tissue; use only the range and timing prescribed by your physiotherapist.

When can I run again?

Running is usually introduced after walking, strength and movement control meet the milestones set by your clinician, followed by a gradual build-up.
