Recognising the pattern
PMDD is a severe premenstrual condition. Physical symptoms often appear during the phase before bleeding, then improve after the period starts, although the pattern differs between people. Abdominal cramps, swelling, breast soreness, headaches, appetite changes, sleep disturbance and marked tiredness may occur. Emotional symptoms such as irritability, anxiety or low mood can be present too.
The timing matters: symptoms that continue throughout the cycle may have another explanation, including migraine, thyroid disease, anaemia, depression or another premenstrual disorder. A symptom diary covering several cycles can help a clinician compare symptoms with your period dates.

Record physical and emotional symptoms each day, including their effect on work, relationships and sleep. Gentle exercise, regular meals, adequate rest and reducing personal triggers may make difficult days more manageable. A clinician can discuss options such as the combined pill, other medicines or psychological support. Do not start or stop treatment without checking how it fits your medical history and other medicines.

Book an appointment if cyclical symptoms disrupt daily life, cause severe pain or make it difficult to function. Seek urgent help for sudden severe headache, chest pain, breathing trouble, fainting or severe abdominal pain.
