What should you know about a PET scan and lung nodules?

Short answer
A PET scan can show activity within a lung nodule, but activity alone cannot determine whether the nodule is cancerous.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- CT assesses
- Nodule structure and location
- PET assesses
- Tracer uptake and activity
- A scan finding
- Does not by itself diagnose cancer
What PET and CT contribute
CT describes a nodule’s size, shape, position and relationship to nearby structures. PET adds information about tracer uptake, which reflects biological activity. Infection and inflammation can also be active, while some cancers may show limited uptake, so the images need review with the nodule’s appearance, earlier scans, symptoms and risk factors.
The report may recommend comparison with older imaging, interval follow-up, a different scan or tissue sampling. A lung nodule is a finding, not a diagnosis, and many possible causes are considered before a plan is chosen.
What to do before and after the scan
Follow instructions about fasting, water, exercise and medicines. Tell the centre about diabetes, recent infection, allergies and any treatment you are receiving. During the uptake period and scan, rest quietly and keep still when asked. Bring prior chest imaging details if requested so the radiologist can compare changes over time.
Ask who will explain the result and whether the next step is surveillance, specialist review or a biopsy. Do not change medicines or delay a recommended follow-up based on the scan wording alone.
When to seek care
Tell staff immediately about sudden breathing difficulty, facial swelling, a widespread rash or faintness after the tracer. Seek urgent help for severe breathlessness, blue lips, coughing blood or collapse. Arrange prompt medical review for new chest pain, persistent worsening cough or rapidly increasing shortness of breath.
Questions for your doctor
What features of the nodule are being assessed on CT and PET? Could infection or inflammation explain the uptake? Are earlier images available for comparison? Is tissue sampling needed, or is planned imaging follow-up appropriate? When should I seek help for breathing or chest symptoms?
Questions people ask
Does PET uptake mean a lung nodule is cancer?
No. Infection and inflammation can also cause uptake, and some cancers may show limited uptake. The full clinical picture matters.
Will every lung nodule need a biopsy?
No. The decision depends on the nodule’s features, changes over time, symptoms and the specialist’s assessment.
