Peritonsillar cellulitis: what to know

The inflammation sits in the soft tissue around the tonsil rather than only on the surface of the throat. Pain may be stronger on one side, and the area can feel tight when you swallow. A muffled voice, unpleasant breath, or trouble opening the mouth can occur. This condition is different from ordinary skin cellulitis, although both involve infected tissue. It can sometimes progress to a pocket of pus, called a peritonsillar abscess.

Arrange prompt medical assessment, especially when swallowing is becoming harder or symptoms are worsening. A clinician may examine the throat and prescribe treatment for the infection. Take prescribed medicine exactly as directed and keep drinking small, frequent amounts if you can swallow safely. Rest, avoid smoking, and do not try to puncture or drain the swollen area yourself. People with diabetes or other immune concerns should mention this during assessment.

Seek emergency care for difficulty breathing, inability to swallow saliva, drooling, rapidly increasing throat or neck swelling, confusion, or marked weakness. These signs can indicate that swelling is affecting the airway or that infection is spreading. Fever with shaking chills or feeling suddenly very unwell also needs urgent attention.

Ask whether the swelling is cellulitis or an abscess, which treatment is needed, how to manage pain and fluids, and what changes should prompt a return visit.
