Periductal mastitis treatment: what should you know?

Short answer
Periductal mastitis treatment depends on whether there is inflammation, bacterial infection, a blocked duct, or an abscess requiring drainage and follow-up.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Assessment
- Examination and sometimes ultrasound guide the treatment plan.
- Abscess care
- A pus-filled collection may need drainage as well as infection treatment.
Why treatment is individual
Periductal mastitis can cause pain, redness, swelling, a firm area, nipple discharge, or a collection of pus. Similar symptoms can arise from other breast problems, so treatment starts with an examination and may include ultrasound. The clinician considers whether infection is likely, whether a duct is blocked, and whether fluid has collected.
Antibiotics may be prescribed when bacterial infection is suspected. An abscess often needs drainage as well as treatment for infection. Recurrent disease may require review of the affected duct and additional follow-up rather than repeated self-treatment.
Supporting recovery
Take prescribed medicine exactly as directed and attend the recommended review, even if pain starts to ease. Use a comfortable supportive bra and ask about suitable pain relief. Avoid squeezing the nipple, forceful massage, or trying to drain a lump yourself, because irritation can worsen swelling.
If you are breastfeeding, a lactation consultation can help with latch and milk removal when feeding is painful. Ask the treating clinician whether to continue feeding or expressing and how to protect your comfort during recovery.
When to seek care
Contact your clinician promptly if redness spreads, fever develops, pain worsens, discharge becomes pus-like, or a lump enlarges. Seek urgent care if you feel faint, severely unwell, or notice rapidly increasing swelling. Arrange follow-up for any mass, nipple change, or discharge that remains after treatment.
Questions to ask
Ask what caused the symptoms, whether imaging or drainage is needed, how to take prescribed treatment, and when improvement should be checked. If symptoms recur, ask whether a blocked duct, smoking, or another underlying issue needs attention.
