What should you know about perianal streptococcal cellulitis?

Short answer
Perianal streptococcal cellulitis is a bacterial infection causing painful, bright-red inflammation around the anus and should be assessed promptly by a clinician.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

How it can present
The infection may cause sharply red skin, tenderness, swelling, itching, cracks or pain when passing stool. Fever can occur, but its absence does not rule out infection. A clinician may inspect the skin and take a swab when needed to identify streptococcal bacteria or distinguish this illness from irritation, fungal infection or another cause.
The bacteria can enter through small breaks in the skin. Conditions that damage the skin barrier, including athlete's foot, or illnesses such as type 2 diabetes may affect infection risk or healing. This condition is different from simple chafing and should not be treated solely with cosmetic creams.
Treatment and home care
Arrange medical review promptly. Treatment commonly involves a prescribed antibiotic selected for the suspected or confirmed infection; take it exactly as directed and complete the prescribed course unless the prescriber changes it. Keep the area gently clean and dry, avoid scratching, and use soft clothing to reduce friction.
Do not share towels, and wash hands after touching the area. Tell the clinician if symptoms are not improving, if you cannot take the medicine, or if you develop a reaction such as facial swelling or breathing difficulty.
When urgent help is needed
Seek urgent care for rapidly spreading redness, severe or escalating pain, marked swelling, confusion, faintness, repeated vomiting, or feeling very unwell. A high or persistent fever, fast breathing or a racing heartbeat can signal a serious infection such as sepsis and needs emergency assessment.
Questions to ask
Ask whether a swab is needed, which antibiotic is being prescribed, how soon improvement should begin, how to protect the skin during bowel movements, and what signs mean you should return urgently. Mention diabetes, recurrent skin infections and any medicine allergies.
