Pedunculated describes shape, not how advanced a melanoma is. The lesion may rise above the skin and can be mistaken for a harmless skin tag or mole. Warning changes can include growth, uneven colour, bleeding, crusting, persistent soreness, or a surface that looks different from nearby skin. Only examination and laboratory analysis of removed tissue can establish the diagnosis and provide details that guide care. A projecting shape should not be used to judge seriousness from appearance alone.

Arrange a prompt dermatology appointment and avoid cutting, tying off, shaving, or repeatedly irritating the lesion. The clinician may examine the entire skin surface and recommend surgical excision so the tissue can be tested. If melanoma is confirmed, treatment planning can include wider removal around the original site and, in selected cases, a sentinel lymph node biopsy. Further care depends on the laboratory findings and whether disease has spread; options may include surgery, immunotherapy, and structured follow-up.

Seek prompt medical review for a new or changing projecting pigmented lesion, especially if it bleeds without clear injury, develops a sore, grows quickly, or changes colour or shape.
