Pearl-like cyst under the skin: what to know

An epidermoid cyst often feels like a rounded lump beneath the surface and may have a small central opening. It can stay quiet for a long time, then become tender, red, swollen, or warm if it ruptures or becomes inflamed. A skin lump has several possible explanations, so appearance alone cannot confirm a cyst.

The clinician may ask how quickly it appeared, whether it drains, whether it changes with pressure, and whether similar lumps have occurred before. Examination is often enough, although unusual or uncertain lesions may need further assessment. Do not squeeze, pierce, or cut it at home, because this can worsen inflammation and introduce infection.

Keep the area clean and avoid friction from tight clothing, shaving, or repeated touching. A warm, clean compress may ease tenderness while you arrange an assessment, but it will not reliably remove the cyst wall. If it is inflamed, treatment may differ from treatment of a quiet lump. Options discussed by a clinician can include an injection into the skin, drainage for a collection of pus, or cyst removal when the whole cyst needs to be taken out.

Arrange prompt medical review if the lump becomes rapidly larger, painful, red, hot, or starts draining foul-smelling material. Fever, spreading redness, red streaks, or feeling unwell can indicate a more serious infection and warrant urgent assessment. Seek care for a firm lump that is fixed, repeatedly returning, bleeding, ulcerating, or changing in colour or shape.
