Pathophysiology of hand, foot and mouth disease

After the virus enters the body, it multiplies in the throat and gut and can spread through close contact, respiratory droplets, saliva, stool and fluid from blisters. The immune response may cause fever, reduced appetite and tiredness. Small painful sores can appear in the mouth, while red spots or fluid-filled blisters may develop on the hands and feet; the buttocks or legs can also be involved. Mouth pain may make drinking difficult, especially for young children. The illness is usually self-limited, but contagiousness can continue when symptoms are improving.

Offer frequent sips of cool fluid and soft foods if mouth sores hurt. Use only age-appropriate fever or pain medicine according to a clinician's or pharmacist's advice; never give aspirin to a child unless specifically directed. Wash hands after nappy changes and toilet use, clean shared surfaces, and avoid sharing cups, cutlery or towels. Keep the child home from childcare or school while they are unwell and follow local guidance about returning. Arrange prompt medical assessment if a child cannot drink, is passing very little urine, has a dry mouth, no tears, unusual drowsiness, breathing difficulty, a seizure or a fever that concerns you. Seek urgent help for a severe headache with neck stiffness, confusion, weakness or a rapidly worsening child. These signs may indicate dehydration or another illness rather than an uncomplicated rash.
