Testing for patellar tendon rupture combines a physical examination of active knee extension with imaging when the diagnosis is uncertain. The clinician asks how the injury happened, looks for swelling or a tendon gap, and checks whether you can lift the straight leg. The examination is adapted if pain or swelling makes movement unsafe.

What the examination involves

You may be asked to tighten the thigh and lift the leg while keeping the knee straight. The clinician compares this with the other side and feels around the kneecap and tendon. A preserved straight-leg raise does not always exclude a smaller tear, so pain, strength, alignment, and the injury mechanism also matter.

X-rays can show a fracture, kneecap displacement, or other bony clues. Ultrasound can assess tendon continuity at the bedside. Magnetic resonance imaging may be used when the tear level, extent, or other soft-tissue injury remains unclear. No single test replaces a careful examination.

Before and after testing

Keep the knee supported and avoid repeated straight-leg tests, squats, stairs, and forceful stretching until assessed. Tell the clinician about prior knee surgery, medicines, wounds, and any pop or fall. If a rupture is confirmed, the next discussion may include Tendon Repair Surgery and protection in a brace.

These signs need rapid evaluation beyond a routine appointment.
