Understanding bedtime panic

An episode may include chest tightness, a sense of danger, sweating, trembling, nausea, breathlessness, or tingling. Worry about having another attack can keep the alarm response active and make sleep harder. Bedtime episodes can also overlap with reflux, asthma, sleep problems, medication effects, or changes in caffeine or alcohol use. Recurrent unexpected attacks together with ongoing concern or avoidance can be features of Panic Disorder, but a clinician must assess the full picture.

A calmer bedtime response

If panic rises, sit up, place both feet on the floor, and use slow, comfortable breathing rather than forcing deep breaths. Keep the room softly lit, focus on a neutral object, and describe the sensations without judging them. If sleep does not return, move briefly to a quiet activity and return when drowsy. Keep regular sleep and wake times, reduce late stimulants, and record episodes, food, medicines, and sleep quality.

When urgent help is needed

Seek emergency care for severe or new chest pain, serious breathing difficulty, fainting, confusion, or symptoms that suggest a stroke.
