What should you know about a painful skin tag under the tongue?

The tissue under the tongue includes the frenulum, salivary gland openings, and delicate mucosa. A sore flap may follow a bite, sharp tooth, dental appliance, or repeated rubbing. A mouth ulcer can be painful without being a skin tag. A blocked salivary duct may cause swelling or discomfort that changes around meals. Infection can cause increasing tenderness, redness, discharge, or swelling. A persistent, firm, bleeding, or ulcerated area needs direct inspection.

Because this is an oral site, a dentist, oral-medicine clinician, or doctor can usually assess the surface and nearby structures more safely than home treatment can.

Rinse gently with plain water, choose soft foods if chewing hurts, and avoid spicy or acidic items that sting. Check for a sharp tooth edge or loose dental appliance, but do not file or alter dental work yourself. Do not pull, cut, burn, or tie off the tissue. Keep a note of when it appeared, whether it changes with meals, and any recent bite or dental procedure.

Arrange prompt assessment if the area persists, enlarges, bleeds, becomes firm, or makes eating difficult. Seek urgent care for rapidly increasing tongue or floor-of-mouth swelling, fever, pus, severe pain, or difficulty swallowing. Emergency help is needed if swelling affects breathing or you cannot handle saliva.

Ask whether the tissue is normal anatomy, an ulcer, a salivary-duct problem, or another lesion; ask whether dental correction or testing is needed. Mention smoking, tobacco or vaping use, recent dental work, and any repeated trauma.
