Short answer

A painful lump under the foot needs assessment if it persists, grows, affects walking, or comes with redness, warmth, numbness, or injury.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

What the lump may mean

The sole is exposed to repeated pressure, so a firm area can come from a callus, a thickened band of tissue, or a small fluid-filled structure. A change in foot shape, such as a bunion or hammer toe, can shift pressure onto one spot. Foot pain can also arise from plantar fasciitis, although that condition more often causes aching near the heel than a distinct lump. A recent increase in activity, a hard landing, or a twist raises concern for an injury such as a stress fracture. The appearance, exact location, tenderness, skin changes, and whether it moves help guide an examination.

A clinician may examine your shoes, walking pattern, joints, and skin. Imaging is considered when the lump is deep, the bone is tender, or symptoms followed trauma.

What you can do now

Reduce activities that repeatedly press on the area and choose shoes with a wide toe box and cushioned sole. Do not cut into the lump or use strong acid treatments on broken or uncertain skin. A cool pack wrapped in cloth can ease soreness for a short period. Note when it appeared, whether it changes after standing, and whether there is tingling. Custom Orthotics may help redistribute pressure when foot mechanics contribute, but they should follow an assessment rather than cover a rapidly changing lump.

When to seek care

Arrange prompt medical assessment for a lump that enlarges, becomes hot or red, drains, causes fever, or makes normal walking difficult. Seek urgent care after a significant injury if you cannot bear weight, the foot looks misshapen, or toes become pale, blue, cold, or numb. Persistent pain without a clear cause also deserves an examination.

Questions for your doctor

Ask: What tissue seems to form the lump? Could my footwear or walking pattern be adding pressure? Do I need imaging? Which activities and pain-relief options are suitable while it is being assessed?

Questions people ask

Can a callus feel like a lump under the foot?

Yes. Repeated pressure can thicken the outer skin and make a tender, firm spot, but a deeper lump should be checked rather than assumed to be a callus.

Should I keep exercising?

Choose low-impact activity that does not increase pain, and stop if walking becomes altered or the lump becomes more swollen.
