What it may mean

A muscle knot can feel like a firm tender spot that hurts when pressed or when the calf contracts. It may follow unfamiliar exercise, prolonged standing, dehydration, a sudden increase in walking, or compensation for pain elsewhere. Myofascial pain syndrome can include trigger points in different muscles and may coexist with lower back, neck or upper-back pain. A calf lump is not automatically muscular: bruising, a muscle strain, tendon trouble or a blood clot can produce different patterns. One-sided swelling, warmth and tenderness are especially important clues.

What you can do now

Pause the activity that started the pain and keep the ankle moving gently if movement is comfortable. Do not force deep massage over a new lump or sharply painful area. Hydration, comfortable walking and a gradual return to activity may help an uncomplicated tight muscle. A physiotherapist can examine strength and movement and may consider manual therapy, myofascial release, trigger point therapy or dry needling when appropriate. Treatment should follow an assessment rather than assuming every lump is a knot.

When to seek care

Seek urgent medical care for sudden one-sided calf swelling, warmth, redness, unexplained tenderness, or pain after prolonged immobility, especially with breathlessness or chest pain. Prompt assessment is also needed after a forceful injury, if you felt a snap, or if walking is difficult. Do not massage a swollen calf until a clinician has advised that it is safe.
