Pain when having sex during ovulation

Ovulation can cause one-sided pelvic discomfort, and deep penetration may make that sensitivity more noticeable. Pain at the vaginal opening, burning, or stinging suggests a different pattern from pain deep in the pelvis. Deep pain that recurs with periods or bowel movements can occur with Endometriosis or an Ovarian Endometrioma. Discharge, fever, bleeding after sex, or pelvic tenderness can occur with Pelvic Inflammatory Disease. Dryness and fragile tissue after menopause can occur with Genitourinary Syndrome of Menopause. Vulvodynia and Lichen Sclerosus can cause external pain.

Stop if sex hurts, communicate about depth and position, and consider lubricant if dryness contributes. Record where the pain is, its relation to ovulation and periods, and whether bleeding, discharge, urinary symptoms, or bowel pain occur. Arrange a gynaecology review when pain repeats, limits intimacy, or does not settle. Seek urgent care for sudden severe pelvic pain, fainting, shoulder-tip pain, heavy bleeding, fever with pelvic pain, vomiting, or a possible pregnancy with pain or bleeding. Prompt review is also needed for new sores, marked vulval swelling, foul-smelling discharge, or pain after sexual assault.

Ask whether the pain is superficial or deep; whether examination, infection testing, or imaging is appropriate; how ovulation timing is being assessed; and what treatment can protect comfort and sexual wellbeing.
