Pain on the left side of the stomach during ovulation

Ovulation pain, also called mittelschmerz, may occur when an ovary releases an egg. It can feel like a twinge, cramp, pressure, or sharper ache on one side of the lower abdomen. The side may vary from cycle to cycle, and the timing may be just before, during, or shortly after ovulation.

Pain in this area can also come from bowel or urinary problems, period-related cramping, pelvic muscle strain, endometriosis, or an ovarian cyst. The location alone cannot identify the cause. Note when the pain starts, how long it lasts, whether it changes with movement, and whether bleeding, discharge, fever, nausea, or urinary symptoms occur.

If the discomfort is mild, rest, gentle movement, and a warm compress over the lower abdomen may help. Keep a cycle diary with the date, side, intensity, and associated symptoms. This can show whether the pattern repeatedly matches ovulation.

Speak with a clinician if the pain keeps returning, interferes with daily activities, or differs from your usual pattern. They may ask about your cycle and pregnancy possibility, examine your abdomen and pelvis, and decide whether testing such as an ultrasound is appropriate. Do not take a medicine that may be unsafe for you without checking its label or asking a pharmacist.

Seek urgent care for sudden or intense pain, fainting, marked weakness, repeated vomiting, a swollen or rigid abdomen, heavy vaginal bleeding, fever, or pain with a possible pregnancy. These symptoms can signal a problem that needs prompt examination.

Arrange a routine appointment when pain lasts beyond the expected mid-cycle window, steadily worsens, occurs with sex, or is accompanied by unusual bleeding or discharge. A clinician can check for causes such as endometriosis or ovarian cysts without assuming that one-sided pain identifies either condition.
