Pain on the inside of the lower leg: what to know

Short answer
Pain on the inside of the lower leg may reflect muscle or tendon overload, an ankle injury, or irritation around the shin and needs assessment if it persists or limits walking.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

What this location can mean
The inner lower leg includes the shinbone, muscles, tendons and tissues that support the ankle. Pain after a new workout, longer walk or change of footwear can fit an overuse pattern. Tenderness near the ankle may accompany an ankle sprain or tendon irritation; pain after a twist, fall or direct blow raises concern for a more substantial injury. Stiffness and swelling can occur with ankle osteoarthritis. A fracture is more concerning when pain is sharply localised, walking is difficult, or the area is swollen after trauma. The symptom alone cannot identify one condition.
What you can do now
Reduce running, jumping and hills for a short period, while keeping gentle movement that does not increase pain. Elevate the leg if it feels swollen, and use a cold pack wrapped in cloth for brief periods during the first day after a minor injury. Supportive shoes may reduce strain. Do not push through worsening pain or repeatedly test a tender spot. A clinician or physiotherapist can compare both legs, check the ankle and assess walking mechanics; treatment may include graded strengthening and a gradual return to activity. See Sports Physiotherapy for activity-specific rehabilitation.
When to seek care
Arrange prompt medical assessment after a significant injury, if you cannot bear weight, or if swelling, bruising or tenderness is increasing. Seek urgent help for a visibly deformed leg, a foot that becomes cold or unusually pale, new weakness or numbness, or severe pain that is rapidly worsening. Book an appointment when symptoms keep returning, disturb sleep, or do not settle with reduced activity. An examination may determine whether imaging or a focused ankle assessment is needed.
Questions for your doctor
Ask: Which structure seems painful on examination? Should I change walking, running or footwear? What activity is safe while it settles? Do I need imaging, and which exercises should I begin first?

Questions people ask
Can pain on the inside of the lower leg come from the ankle?
Yes. An ankle sprain, tendon irritation or joint stiffness can be felt above or beside the ankle rather than only at the joint.
Should I keep exercising with this pain?
Switch to gentle, comfortable movement and pause impact activity if pain increases during exercise or lingers afterward.
