Pain on both sides during ovulation

Pain on both sides during ovulation can reflect mid-cycle pelvic irritation, but ovarian cysts, endometriosis, or another cause may feel similar. Ovulation pain is often brief, yet the location on both sides does not prove that both ovaries released an egg.

Fluid or mild irritation around ovulation can be felt across the lower pelvis rather than at one precise spot. The bowel, bladder, abdominal wall, and pelvic-floor muscles can also refer discomfort to both sides. Endometriosis may cause cycle-linked pain, and an ovarian cyst may cause pressure or aching on one side or across the pelvis. Pain that lasts beyond the expected mid-cycle window, happens repeatedly, or is associated with painful periods or pain during sex warrants assessment.

Track the first and last day of pain alongside cycle dates, bleeding, bowel and bladder symptoms, and any pain during sex. Rest, gentle heat, hydration, and a pain medicine you can safely use may provide relief. Book a gynaecology review when the pain is recurrent, worsening, or disruptive. A clinician may consider an examination, pregnancy testing, or ultrasound based on the pattern.

Seek urgent care for sudden severe pain, fainting, heavy bleeding, fever, repeated vomiting, a swollen abdomen, or marked weakness. Get prompt advice if pain occurs with a positive pregnancy test or becomes rapidly worse.

Could the pain be from ovulation, an ovarian cyst, endometriosis, or the abdominal wall? What duration or pattern would make imaging useful? Which pain relief is suitable if pregnancy is possible?
