Pain near the cervix during sex

Pain near the cervix during sex is usually deep pelvic pain triggered by penetration and can have muscular, hormonal, infectious, or gynaecological causes. A deeper position, limited lubrication, or tense pelvic-floor muscles may trigger it, but recurring pain should be assessed.

Endometriosis, an ovarian endometrioma, pelvic inflammatory disease, and other pelvic conditions can cause deep pain with sex. Genitourinary syndrome of menopause may make vaginal tissues drier and more sensitive. Vulvodynia and lichen sclerosus more often cause pain at the vulva, although pain can feel difficult to localise. Burning, unusual discharge, bleeding after sex, fever, urinary symptoms, or pain outside sex can help a clinician narrow the possibilities.

Stop or change activity when pain begins; continuing through pain can increase muscle tightening. Try slower penetration, positions that limit depth, and a suitable lubricant. Arrange an appointment if the symptom repeats, affects intimacy, or comes with discharge or bleeding. Assessment may include a gentle pelvic examination, infection testing, and discussion of periods, contraception, menopause symptoms, and bowel or bladder changes.

Seek prompt medical care for severe or sudden pelvic pain, fever, fainting, heavy bleeding, vomiting, or feeling acutely unwell. Contact a clinician soon for new bleeding after sex, foul-smelling discharge, or pain with a possible pregnancy.
