Lower abdominal pain during ovulation: what to know

Ovulation pain can feel like a brief cramp, pressure, or sharp twinge low in the abdomen as an ovary releases an egg. It may be felt on one side, in the centre, or across the pelvis, and its timing can vary. Bowel symptoms, urinary irritation, pelvic muscle tension, endometriosis, and ovarian cysts can mimic it. Endometriosis may cause cycle-linked pelvic pain. Ovarian cysts may cause aching or pressure and can sometimes produce sudden symptoms. The location alone does not establish the cause.

Track the cycle day, exact location, duration, severity, and any bleeding, discharge, nausea, fever, bowel, or urinary changes. Gentle heat, rest, fluids, and comfortable movement may ease mild symptoms. Take over-the-counter pain relief only if it is safe for you and use the label directions. If pregnancy is possible, test and seek advice about medicines. Make a routine appointment if the pain returns, lasts longer than expected, or disrupts work, sleep, or movement.

Seek urgent care for sudden severe pain, rapid worsening, fainting, marked dizziness, fever, repeated vomiting, heavy bleeding, or a rigid or swollen abdomen. One-sided pain with a missed period or positive pregnancy test also needs prompt assessment because an ectopic pregnancy can be dangerous.

Ask whether the timing fits ovulation, whether an examination, urine or pregnancy test, or ultrasound is appropriate, and what symptoms should trigger urgent care. Mention any previous cysts, endometriosis, pelvic surgery, or fertility treatment.
