Pain near the cervix during sex: what to know

Deep pain may happen when penetration reaches the cervix or puts pressure on inflamed pelvic tissues. Position, depth, insufficient lubrication, and pelvic-floor muscle tightening can contribute. Endometriosis, an ovarian endometrioma, or pelvic inflammatory disease may cause deeper pelvic pain. Genitourinary Syndrome of Menopause can cause dryness and tissue sensitivity. Lichen sclerosus and vulvodynia more often affect the vulval area but can make sexual activity painful. The sensation does not identify one cause by itself.

Pause when pain starts and try a position with less depth or more control. Use a suitable lubricant, allow more arousal, and avoid scented products that irritate sensitive tissue. Note whether pain relates to your cycle, penetration depth, bleeding, discharge, urinary symptoms, or dryness. Arrange a clinical assessment if it repeats or affects your relationship or wellbeing. Examination and targeted tests can distinguish infection, tissue changes, muscle tension, and pelvic conditions.

Seek urgent care for severe or sudden pain, fainting, fever, repeated vomiting, heavy bleeding, or foul-smelling discharge. Prompt review is needed for pain with a missed period or possible pregnancy, or for severe symptoms after a pelvic procedure.

Ask whether the pain is likely from contact, muscles, dryness, infection, or a deeper pelvic condition; whether examination or imaging is useful; and how to keep intimacy comfortable while you are being assessed.
