Pain in the lower belly during sex

Pain in the lower belly during sex may be linked with pelvic muscle tension, irritated tissue, infection, or an ovarian condition. Repeated or lingering pain should be assessed rather than treated as something to endure.

Pain with deep movement can occur when the cervix, uterus, ovaries, or surrounding tissues are sensitive. Endometriosis and ovarian endometrioma may cause deep intercourse pain with period-related symptoms. Pelvic inflammatory disease may cause tenderness with unusual discharge, fever, or bleeding. Genitourinary syndrome of menopause can cause dryness and burning, while vulvodynia or lichen sclerosus can make external contact painful.

These are possibilities, not conclusions. Tell a clinician whether pain is deep or near the opening, one-sided or central, sudden or gradual, and whether it continues after sex.

Stop if the pain becomes uncomfortable and consider gentler, shallower activity only if it does not hurt. Keep notes about cycle timing, positions, bleeding, discharge, fever, bowel symptoms, and urinary symptoms. A medical assessment may include an abdominal and pelvic examination, with infection, pregnancy, or ultrasound testing when appropriate. Care is tailored to the cause and may include pelvic floor therapy, tissue care, infection treatment, or treatment for a pelvic condition.

Get urgent care for severe sudden pain, fainting, heavy bleeding, fever, repeated vomiting, or possible pregnancy with pain. Seek a timely appointment if the problem recurs, interferes with intimacy, persists afterwards, or comes with bleeding after sex or new discharge.
