Pain in Lower Abdomen During and After Sex

The timing and character of the pain help guide the next step: pain with deep penetration can differ from burning at the vaginal opening, cramping that continues afterwards, or sudden pain on one side. A clinician may ask about bleeding, discharge, urinary symptoms, bowel symptoms, menstrual changes, pregnancy possibility, and whether the pain is new or recurring.

Deep pelvic pain can occur with endometriosis, an ovarian endometrioma, or another ovarian problem. Endometriosis may also be associated with painful periods or pain with bowel movements. Pelvic inflammatory disease can cause lower abdominal pain with unusual discharge, bleeding between periods, fever, or pain when the cervix is moved during an examination.

Low oestrogen around menopause can make vaginal tissues drier and more sensitive; genitourinary syndrome of menopause is one possible explanation. Tight or overactive pelvic floor muscles can make penetration painful and leave aching afterwards. Skin conditions such as lichen sclerosus and nerve-related pain such as vulvodynia more often cause soreness, burning, or pain near the vulva, but discomfort can extend into the pelvic area.

Pause penetration if it causes pain, and record when the pain starts, where it is, how long it lasts, and whether it follows a particular position. Note any bleeding, discharge, fever, nausea, urinary changes, bowel symptoms, or relation to your menstrual cycle. Do not use pain as a reason to push through sex.

Arrange a medical assessment if the pain is new, keeps returning, or affects intimacy or daily activities. Depending on your symptoms, assessment may include a pregnancy test, infection testing, a pelvic examination, or imaging. Treatment is directed at the cause and may include treating an infection, addressing vaginal dryness, or pelvic floor physiotherapy. Surgery is not automatically needed; when endometriosis is confirmed and symptoms persist, laparoscopic excision may be discussed.

Seek urgent medical care for severe or rapidly worsening pain, fainting, marked dizziness, heavy bleeding, fever with pelvic pain, repeated vomiting, or a rigid or very tender abdomen. If pregnancy is possible, pain with bleeding, shoulder-tip pain, weakness, or fainting needs urgent assessment because an ectopic pregnancy can be dangerous.

Prompt assessment is also appropriate for pain with fever or foul-smelling discharge, new pain after a possible sexually transmitted infection exposure, or sudden one-sided pain with nausea. These patterns can signal infection or an ovarian emergency and should not be judged by how common the underlying cause is.

Ask: Could the pain be related to an infection, endometriosis, an ovarian condition, pelvic floor muscles, or vaginal tissue changes? What examinations or tests fit my symptoms? Which symptoms mean I should seek urgent help? What can I do safely while waiting for assessment, and should my partner be tested or treated?
