Lower abdominal pain before ovulation: what to know

Some people feel a cramp or pulling sensation as the ovaries and surrounding tissues change during a cycle. Ovulation timing varies, so pain before an expected fertile window does not prove that ovulation is the cause. Endometriosis may cause cyclical pelvic pain, painful periods, painful sex or bowel discomfort. An ovarian cyst can cause pressure or one-sided pain, while constipation, urinary infection and abdominal muscle strain may produce lower abdominal symptoms too. A pregnancy-related cause must be considered when pregnancy is possible.

Keep a cycle diary showing the first day of bleeding, pain location, triggers, bowel and urinary symptoms, and how long discomfort lasts. Gentle heat, rest and an appropriate pain-relief option may be useful; check with a pharmacist if you could be pregnant or have medical conditions. Arrange a clinical review for repeated episodes, pain that interferes with daily life, or symptoms that do not follow a familiar pattern.

Get urgent help for severe or rapidly increasing pain, fainting, heavy vaginal bleeding, fever, repeated vomiting or pain with a positive pregnancy test. These warning signs can accompany an ovarian emergency, pelvic infection or pregnancy-related complication. Seek prompt review for new pain with unusual discharge, painful urination, blood in the stool or persistent tenderness.
