Short answer

Pain in the left forearm and chest can come from muscles or digestion, but heart and lung conditions can also present this way and require urgent assessment.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Priority
New chest pain spreading to an arm needs prompt assessment
Warning signs
Breathlessness, sweating, nausea, faintness, weakness, or coughing blood
Possible sources
Heart, lungs, digestive tract, nerves, muscles, or skin

Possible explanations

Chest discomfort spreading into the arm can occur with angina, which is related to reduced blood flow to the heart. A heart problem may feel like pressure, squeezing, heaviness, burning, or unexplained discomfort rather than a sharp pain. Lung problems such as pneumonia or pulmonary embolism can cause chest pain, particularly with breathing, and may bring cough or breathlessness. Acid reflux, gastritis, or a peptic ulcer can cause upper abdominal or chest burning. Panic disorder can cause chest discomfort with intense fear, rapid breathing, and a racing heartbeat. Shingles may cause burning or sensitivity before a skin rash. Muscle or joint irritation is another possibility, especially after exertion or injury.

What to do now

Stop exertion, sit upright, and note when the pain began and whether it is spreading. Do not drive yourself if the discomfort is severe or accompanied by breathlessness, sweating, nausea, weakness, or faintness. Contact emergency services for concerning symptoms. Even if the pain settles, arrange prompt medical advice when it is new, recurrent, or triggered by activity. A clinician may use an examination and heart or lung tests to determine the cause.

Get emergency help

Call emergency services immediately for chest pressure or tightness that spreads to the arm, jaw, back, or shoulder; severe breathing difficulty; fainting; cold sweating; sudden weakness; confusion; or coughing blood. Do not wait to see whether an antacid or rest makes it disappear. Chest pain after a major injury, or chest pain with a new irregular heartbeat, also needs urgent attention.

Questions to discuss

Ask which causes fit the timing and triggers, whether you need an ECG or other tests, how to manage risk factors, and which symptoms should lead you straight to emergency care. Tell the clinician about medicines, smoking, pregnancy, recent travel, injury, and previous heart or lung problems.

Questions people ask

Can a pulled muscle cause chest and forearm pain?

Yes, especially after lifting or an injury, but the combination should not be assumed to be muscular when chest symptoms are new or unexplained.

What should I do if the pain has stopped?

Seek prompt medical advice if it was new, spread to the arm, occurred with exertion, or came with breathlessness or other warning signs.
