What should you know about pain in left flank and under ribs?

The area beneath the left ribs overlaps the flank and upper abdomen. Muscle or rib-area pain may worsen with twisting, lifting, deep breathing or touch. Kidney stones can cause intense, wave-like flank pain that may move toward the groin. A kidney infection can present with flank pain plus fever, chills, nausea or painful urination. A urinary tract infection may cause burning and urgency, while pain under the ribs can also arise from digestive or other abdominal structures. The position alone cannot identify the cause.

Rest from strenuous activity and observe whether posture, movement, meals, breathing or touch changes the pain. Record its onset, severity, duration, fever, urine changes, vomiting, bowel symptoms and any recent injury. Maintain usual fluid intake unless you have been advised to restrict fluids. Arrange a medical review when pain persists, returns, or has urinary or digestive symptoms; examination and urine testing may help determine the next step.

Seek urgent care for severe or sudden pain, fever with shaking chills, repeated vomiting, fainting, confusion, blood in the urine, or inability to pass urine. After an injury, emergency assessment is needed for worsening pain, weakness, abdominal swelling or breathing difficulty. Pregnancy with significant flank pain or fever should also be assessed promptly.

Ask whether the pain pattern fits a muscle, rib, kidney or urinary cause; whether urine testing or imaging is needed; which pain relief is suitable; and what warning signs should prompt immediate reassessment.
